Learning objectives: Complete Androgen Insensitivity (XY women)   is present in   08 % to 2, 4 %   in all phenotypic female   infants operated   for   inguinal hernia but this condition sometimes  is  not suspected   by the surgeon . Various siblings may be affected as it is   a genetic   recessive   condition.

Case information:  Three   sisters were operated in their infancy   (each of them by a different surgeon in the same institution) for   incarcerated inguinal hernia.  When teens  a consultation was required due to   primary amenorrhea.   It was then discovered that the three sisters had CAIS. The family presented a claim against the three intervening   surgeons for no timely diagnosis. (Gonadectomy is an indication to prevent malignancies

Discussion: Hernia repair is the most common procedure in pediatric surgery being performed by residents in training and experimented surgeons. Every time that a CAIS is found it looks like an unexpected surprise that contradicts the mentioned incidence It has received little attention in the literature   .  Considering how many hernias shall operate a pediatric surgeon during his/her career is important  remembering   that condition.. Attention must be drawn to the family   backgrounds   ,  and  perform  proper inspection of the gonad if incarcerated (fallopian tubes must be seen). Previous assessment by measuring the length of the vagina has bees proposed  as well as the benefits of laparoscopic intervention that allows full inspection
